About the Role:

Are you passionate about making a difference in people's lives? We are looking for a dedicated and compassionate Reablement Practitioner to join our dynamic team. In this role, you will be responsible for the daily management and supervision of reablement support workers, ensuring that clients receive high-quality, person-centred care that enables them to stay in their own homes and facilitates timely hospital discharge.

The assessment process with clients is a critical part of this role. You will be the first point of contact for new referrals to Reablement this involves Comprehensive Holistic Assessment.  Ensuring the client's home environment supports their independence goals.

Key Responsibilities:

  • Coordinate referrals and assessments to develop personalised goal/care plans.
  • Completion of Medication assessments, production of MAR charts or provision of TEC
  • Conduct comprehensive risk assessments and home checklists to support clients' independence goals.
  • Supervise and support reablement support workers, ensuring their development and compliance with training.
  • Work collaboratively with a multidisciplinary team, including GPs, nurses, occupational therapists, and social workers.
  • Maintain accurate records and ensure compliance with CQC standards and safeguarding procedures.

All about you:

Knowledge of good care practice principles and direct care of older people/people with disabilities.
Excellent interpersonal, written, and verbal communication skills.
Ability to complete assessments, including risk assessments, and develop goal-setting plans.
Experience in staff supervision and performance monitoring.
NVQ III or Health and Social Care Diploma or equivalent qualification.
