Peterborough City Council's Reablement Team are looking for an individual to join our new 1 year Learning Disability Project. The aim of this is to maximise individual's wellbing and independent living skills. As a care support worker in this project you will work closely with people to support them through their reablement time and work with colleagues to support individuals to achieve their goals. This is an exciting opportunity to be part of a new team, making a difference to people with a learning disability and those living with a neurodivergence.
The people on your caseload will all have a Learning Disability (LD) and may include those with complex communication difficulties, sensory needs, and behaviours of concern, health and social care needs and physical disabilities. The work is all based in the community, so it is important that you have your own transport to be able to travel from one location to another.
You will have responsibility & line management for Care Support Workers working in the same area. In Peterborough we work as a multidisciplinary team and are supported by psychology, nursing, speech & language therapy and social workers
You will also have the support the Reablement Team Manager and a Community Long Term Team Manager in service development and management of the day to day running of the LD Reablement Service.
All about You
You are a compassionate and proactive individual with a Bachelor's or Master's degree in Occupational Therapy and are registered with the HCPC. You have experience working as a qualified Occupational Therapist and your strong communication and interpersonal skills enable you to work effectively both independently and as part of a team. You are committed to continuous professional development and are passionate about making a positive impact on the lives of others.
You will have transferrable skills that you can bring from your present role and ensure that your interventions are as holistic as possible taking into account all aspects of the person's daily life.
You will have independent, well-reasoned judgement and decision making skills and excellent communication skills.
You will use highly specialist skills to assess the impact of an individual's learning disabilities on their occupational performance through the use of specific OT & Adult Social Care standardised and non-standardised assessments.
You will implement, monitor and evaluate highly specialist treatment programmes to get the best outcomes.
You will work collaboratively with other health and social care professionals to deliver holistic assessments and improve the quality of life for the people of Peterborough.
