Are you an experienced leader ready to make a difference in adult social care?
We are looking for a Team Manager to lead our Continued Healthcare (CHC) services and Discharge to Assess pathway. This is a pivotal role focused on effective CHC coordination across our system. You will manage an Adult Social Care team supporting people discharged from hospital on a D2A Pathway and those referred to Adult Social Care for participation in the Continuing Health Care process.
Working with the Head of Service, you will drive performance and improve outcomes for residents. You will also ensure strong collaboration with health partners and robust CHC processes.

All about You
Strong knowledge of current legislation and national policy
· Experience in hospital discharge, CHC, or integrated care
· Willing to learn and maintain your professional development
· Excellent communication, negotiation and partnership skills
· Confident with data, budgets, and leading change in a complex system
· Resilient, proactive, and focused on person-centred care
