All about the Role 

An exciting opportunity has arisen to join Peterborough City Council's 18–25 Transition Team as a Team Support Worker.

We are looking for someone passionate about making a difference in the lives of young people as they move from childhood into adulthood. This is your chance to be part of a dynamic, supportive team that works alongside Social Workers to deliver the very best outcomes for young people and their families.

We give young people and their families choice, control, and flexibility in shaping their support. As a Team Support Worker, you'll champion their voices through a Person-Centred Planning approach, ensuring every view, wish, and feeling is heard.

The role involves completing Care Act 2014 Assessments, Reviews, Care and support plans, Mental Capacity Act 2005 Assessments and Best interest Decisions (where applicable) identifying individuals needs and supporting the Multidisciplinary teams' recommendation on eligibility for full NHS Continuing Healthcare. 

The successful candidate will assist families to access activities and services, Advice, advocating and will need to be able to travel independently between locations including out of county areas on occasions.

The 18–25 Transitions Team is part of the Adult Social Care workforce, working closely with the Children with Disabilities Team  (CWD) to assess young people from the age of 17 as they prepare for adulthood. All about You - Essential Criteria for the role

We're looking for someone who is enthusiastic about supporting young people as they transition into adulthood. You'll bring energy, empathy, and commitment to helping families navigate this important stage of life.

To be successful in this role, you will have experience or knowledge of working with young people, and an understanding of the legislation and frameworks that guide this work, including the Care Act 2014, the Children's Act 1989, the SEND Agenda, and the National Framework for Children and Young People's Continuing Care (2016).

Understanding of safeguarding and how to keep vulnerable adults safe.

Knowledge of person-centred care and good customer service.

Basic IT skills (email, word processing, databases).

Ability to organise and prioritise work effectively.

Experience working directly with adults who use social care services.

You'll be confident in completing assessments and care plans, advocating for families, and working collaboratively with multidisciplinary teams.

Strong communication skills, a person-centred approach, and the ability to travel independently are essential.

Most importantly, you'll be someone who values choice, inclusion, and empowerment — ensuring that young people and their families feel heard, respected, and supported every step of the way.
