All about the Role

This is a part time  post of 34 hours 

Are you passionate about person-centred care and team working to support people to live their best lives at home and maximising individuals' independence?  Then this Therapy Caseworker position sounds like the right opportunity for you!

As a Therapy Caseworker, working in the Therapy Services Team within Adult Social Care, you will work directly with people in their homes, to complete holistic assessments and develop practical, person‑centred plans that promote independence, safety and wellbeing.

You will manage your own caseload, identifying needs related to health, housing and daily living, and recommending solutions ranging from equipment and minor adaptations to major home adaptations via Disabled Facilities Grant applications. Working under the clinical supervision of an Occupational Therapist, you'll use your problem‑solving skills to analyse assessment information, make informed decisions, and support clients through the adaptations process. You'll also carry out reviews, liaise with health and social care professionals, maintain accurate documentation, and contribute to the team's wider early intervention and prevention approach, ensuring seamless, compassionate support for vulnerable adults across Peterborough.

Please note the salary advertised, is for a full time post so will be pro rata accordingly, as this is a part time post.

We operate a flexible way of working over the week, you will manage your own diary and there is no weekend working. We are mostly agile and work from home, whilst using the office as an official work base.

You're able to build effective relationships with professionals, families and partner organisations, using your problem‑solving abilities and initiative to deliver person‑centred solutions. You are comfortable working independently and you show empathy, resilience and a positive approach to teamwork. With a basic understanding of occupational therapy, health conditions and simple adaptations, you're ready to make a real difference to people's independence and wellbeing in their own homes.

Strong communication skills including written, typed and verbal are essential, along with the ability to maintain accurate records, liaise with professionals and families, and work both independently and as part of a team. You'll be adaptable, able to prioritise your workload, handle stressful or sensitive situations, and assist people safely in their homes. Competent IT skills are also required.

You'll have experience working with the public, communicating information effectively, and supporting vulnerable people in a safe and empathetic manner.

You'll hold GCSEs in English and Maths or Science (grade C/4+ or equivalent) and be physically fit for the demands of the role. Please note, it is essential for this role that you must hold a valid driving licence and have access to a car to travel across the Peterborough area.
