All about the Role 

Peterborough City Council's Reablement Team is seeking a motivated and caring individual to join the team as a Care Support Worker.

In this role, you will help maximise individuals' wellbeing and promote their independence by supporting and monitoring clients throughout their reablement journey. 

You will work with clients to support them through their reablement time. You will also work with colleagues to support individuals to achieve their goals and complete our Reablement support workers assessments.

As a Care Support Worker, you will collaborate with colleagues and partner agencies to deliver effective reablement support. You will also undertake Adult Social Care assessments to identify longer-term care and support needs, and carry out six-week reviews to evaluate progress and ensure appropriate ongoing support is in place.

 

This role involves working Monday - Friday 9-5, with no weekend work!!
Please note - Occasionally  you will need to be flexible with start and finish times to fit the needs of the service. 
 
Interviews will be held on 28th and 29th September at Sand Martin House

All about you

 
To be successful in this role, you will need excellent interpersonal and organisational skills, a problem solving and positive attitude alongside knowledge and/or experience of the needs of vulnerable adults. Working knowledge and experience of the Care Act & Eligibility Criteria and Mental Capacity Act is essential.
 

Please note for this role you must hold a Full Uk driving license and access to a vehicle
