All about the Role
We're looking for a Qualified Social Worker to join our Transition Team, supporting young people aged 18–25 as they move from children's to adult services. You'll work directly with young people and their families, completing strengths‑based assessments, care planning, reviews, and safeguarding work in line with the Care Act.
Working closely with colleagues across children's and adult services, health, education, housing, and voluntary partners, you'll help ensure transitions are timely, well‑planned, and focused on each young person's goals as they move into adulthood.

All about You
- You have current registration with Social Work England
- You have completed your AYSE
- You have an in-depth knowledge of current legislation and guidance, as well as current national policy and practice developments
- You have a full Drivers Licence and the ability to travel between different locations
- You are willing to learn new things and able to maintain your own continuing professional development
