All about the Role

We're looking for a Qualified Social Worker to join our Transition Team, supporting young people aged 18–25 as they move from children's to adult services. You'll work directly with young people and their families, completing strengths‑based assessments, care planning, reviews, and safeguarding work in line with the Care Act.

Working closely with colleagues across children's and adult services, health, education, housing, and voluntary partners, you'll help ensure transitions are timely, well‑planned, and focused on each young person's goals as they move into adulthood.

All about our Team

You'll receive regular supervision, opportunities for professional development, and the support you need to thrive in your role.

Our work is dynamic and impactful, and you'll be part of a council that is committed to inclusion, development, and high‑quality practice. If you're looking for a role where you're supported, valued, and able to make a real difference, the Transition Team could be the perfect place for you.
