All about the Role
We are looking for an experienced and motivated Advanced/ Senior Practitioner to join our 18 –25 Transition Team. You will hold a manageable caseload alongside providing professional guidance and supervision to social workers and other team members. You will support the Team Manager with the day-to-day running of the service and provide management cover when required. You will bring strong knowledge of the Care Act 2014, safeguarding adults, the Mental Capacity Act, Court of Protection (CoP) and Deprivation of Liberty Safeguards (DoLS), supporting high-quality, legally compliant practice.
This is a part time role so please note the salary will be pro rata.
The role involves working two days a week. Monday is a mandatory working day in person at Sand Martin House

Our Transition Team supports young people as they prepare for adulthood, working closely with families, carers, education, health and other professionals. We are a supportive and collaborative team committed to achieving positive outcomes for young people while promoting independence, choice and control. As a Senior Practitioner, you will play an important role in supporting consistent practice, developing staff and contributing to continuous service improvement.
All about You
- You will be an experienced, confident and compassionate qualified social worker with strong assessment, safeguarding and decision-making skills.
- You will be able to manage complex cases, provide effective supervision and offer professional challenge and guidance when required.
- Most importantly, you will be passionate about supporting young people through transition and developing colleagues to deliver strengths-based, person-centred and outcome-focused practice.
- Work collaboratively with internal and external partners.
- Contribute to training, development and continuous improvement across the service.
- Support quality assurance, performance monitoring and service improvement.
- You have a full Drivers Licence and the ability to travel between different locations
